Instruction : REQUESTED INQUIRY FOR CONTRACT MODIFICATION: To request a review and consideration regarding a modification to an existing contract, please fill in the names and e-mail addresses for your agency. After pressing “SEND”, they will receive an e-mail notification from DocuSign. In that e-mail, there is a Review Document button that will need to be clicked.
