Welcome to MHSPD

To Licensed Mental Health Professionals,

Santa Clara County Mental Health Department, Department of Alcohol and Drug Services, and Superior Court are creating a web-based resource listing of local licensed mental health professionals and community agencies employing licensed mental health professionals, who declare that they are qualified to provide one or more of the following services:

1.  Family Court Related Counseling and Evaluation Services.
2.  Mental Health Treatment and Counseling Services.
3.  Substance Abuse Treatment and Counseling Services.

Mental Health Professional Services Form

After you submit the on-line Mental Health Provider Services application, please complete the Website Provider Declaration and Agreement form and mail, along with a copy of your current professional license to:

Once your application, agreement form and license have been received by the Mental Health Department, your services will be posted on the MHPSD website.
